Key Responsibilities
  • Initiate client sessions to uncover individual goals, preferences, and options for personalized treatment, tailored to each client’s unique circumstances.
  • Develop and implement individualized rehabilitation plans that account for clients' educational backgrounds, strengths, challenges, and employment aspirations, making revisions as necessary throughout the treatment process.
  • Collaborate closely with a multidisciplinary team of medical professionals, including physicians and occupational therapists, to ensure cohesive and effective rehabilitation services are provided.
  • Maintain meticulous case management records detailing client demographics, eligibility, insurance specifics, services rendered, and communications, in accordance with clinical standards and regulations.
  • Regularly evaluate client progress by monitoring documented outcomes, assessing the effectiveness of treatment plans, and making adjustments as required for optimal results.
  • Actively communicate with clients during their participation in job training and rehabilitation programs, proactively addressing challenges and facilitating successful placements.
  • Engage with clients’ families to develop collaborative strategies that promote effective support systems.
  • Conduct assessments to identify client needs, compiling thorough reports in a timely manner.
  • Guide clients to appropriate resources and provide directional advice to help them navigate their treatment journeys successfully.
Patient Population / Client Focus

Qualifications
