Job description

The LPN is responsible for the well-being and nursing care of all residents assigned during his or her shift. He or she provides nursing services, helps carry out physician orders, assesses residents to note any changes in condition, responds to and reports resident changes in condition, distributes and charts medications, distributes/counts narcotics and maintains narcotic inventory record, and responds to emergencies. In addition, the LPN provides direct supervision for the Caregivers and Medication Technicians on duty to ensure quality and safe delivery of resident care services. Starting Rate $33hr.
Essential Job Functions:
  • Assesses and reassesses residents with respect to appropriateness for Community residency in accordance with State regulations and the resident's safety and comfort.
  • Obtains, reviews and documents resident medial history, physician orders, transfer and pre-admission information and reviews social history. Provides special instructions, as needed, to Food Service Department.
  • Develops individualized service plans for each resident based on resident's needs, condition and other care taking considerations. Tracks resident's responsiveness to the service plan and updates plans, as needed, based on resident conditions.
  • Maintains and updates records, unit charts, medical charts, information forms and communication logs regarding resident service. Audits resident medical files on prescribed basis.
  • Schedules appointments and arranges transportation to other departments or facilities, completing required forms and arranging for appropriate escorts.
  • Inventories emergency medications and checks status of supplies and equipment to ensure unit needs are met, taking action to replace or repair broken or malfunctioning equipment.
  • Responds to emergency needs of residents, procuring treatment, communicating with physicians and families when emergency treatment is needed, and reporting any accidents or unusual incidents as necessary.
  • Supervises care givers on a daily basis. Orients new care givers, schedules staff, provides in-service training, evaluates performance, and coaches care givers on providing service that meets the Community's standard of care.
  • Participates in interviewing candidates and selecting new care givers who will model the Community's service standards.
  • Communicates regularly with residents' health care providers and families, supporting a team approach to managing the health, safety and comfort of each resident.
  • Serves as backup for the Delegating Nurse with responsibility for maintaining all medications, charting medication administration, charting treatment and services provided to residents, and communicating with health care providers regarding specific resident needs.
  • Provides care to individual residents, including medication administration, as needed, to supplement available care giving staff.
  • Monitors resident treatment, medication and diet orders, checking resident's daily schedule and ensuring that care givers meet resident health and personal care needs.
