Job Description
ROLE - SPECIALTY
Schedule:
- Shift: 8-hour day shifts only
Assignment Details:
- Guaranteed Hours: Up to 6 patient visits/day; may call off up to one shift per pay period
Requirements:
- License: Required, with acceptance of pending licenses
- Experience: 1 year of skilled home health experience
- Certifications: BCLS
- EMR: Experience with OASIS documentation expected
Roles & Responsibilities:
- Evaluate and assess patient needs to develop appropriate treatment plans
- Implement home exercise programs and provide patient/family education
- Facilitate post-operative rehabilitation for orthopedic and neurological conditions
- Monitor and document patient progress and adjust treatment plans as necessary
- Utilize safe body mechanics and mobility techniques during patient interactions
Compliance / Notes:
- Dress Code: Business casual attire (scrubs allowed but not required)
- Parking: Free on site
- Communication: Clinician responsible for own communication device if not provided
