Job description

Summit Home Care and Hospice is searching for an experienced and motivated Admission Coordinator to join our team. We're committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.
Admission Coordinator:
  • Hours: Monday - Friday; 8:30 AM - 5:00 PM
  • Location: 800 Yard St. Suite 300, Columbus, OH 43212
  • Work Setting: On-Site 5 Days/Week & Remote After 3 Months
  • Pay Rate: $24.00 - $30.00/Hour

Responsibilities:
  • Responsible for contacting potential patients and physicians regarding services, field intake calls, resolve patient/referral source inquiries, enhance continuity of patient care by providing support to Account Managers and referral sources.
  • Receive, review, and monitor new referrals to determine ability to accept or decline.
  • Accurately determine and anticipate homecare needs and accept and decline on clinical appropriateness.
  • Call for, follow up on, track and obtain verbal orders for admissions.
  • Consistently communicate with the Account Managers and leadership teams regarding new business plans and updates.
  • Provide exceptional customer service to patients.
  • Complete pre-surgery calls and/or confirmation calls.
  • Respond to and initiate intake calls to providers, patient care teams and family to gather necessary patient information solely for the intent to process for care.
  • Process referrals with accuracy, timeliness, and professionalism.
  • Work with ADONs and the Patient Access Representatives teams to ensure timeliness of processing and compilation of required paperwork for each patient.
  • Provide support for Account Managers in the field.
  • Serve and protect the company by adhering to professional standards, home care policies and procedures.
  • Facilitate the coordination of services to support physicians, patients, caregivers, and/or the community.
  • Utilize both Summit's EMR and external EMR systems effectively for accepting, declining and bundling referrals.
  • Meet established KPI's as assigned.
  • All other duties as assigned/directed.

Qualifications & Experience:
  • High School Diploma or equivalent required.
  • LPN/RN highly preferred.
  • Must be able to type 50 WPM.
  • 2+ years of experience in healthcare data.
  • 1+ years of experience in home health admissions preferred.
  • Experience in home health care, durable medical equipment, or skilled nursing facility preferred.
  • Previous experience in customer service.

Skills:
  • Excellent verbal and written communication skills.
  • Ability to build rapport with clients and account management.
  • Ability to work independently and as an active member of a team.
  • Strong computer skills in data entry, and Microsoft applications.
  • Accurate and precise attention to details and deadlines.
  • Ability to multitask, prioritize, and manage time efficiently.
  • Goal-oriented, organized team player.

Pre-Employment Requirements:
  • Must successfully pass a background check in accordance with state and federal regulations.
