Position Summary
The Registered Nurse (RN) Care Manager or Licensed Social Worker (LSW/LISW) Care Manager works collaboratively as part of a care management team to onboard new families onto service and provide ongoing support to caregivers. This role is responsible for assessing consumer and caregiver needs, evaluating eligibility for service, and guiding prospective families through an efficient activation of services. The Nurse or Social Worker establishes a person-centric plan of care, completes an orientation to Careforth's digital communication platform, and conducts regularly scheduled home visits. For an assigned caseload, this position coaches, advocates, and educates caregivers to assist with the daily health and well-being of their care recipients.
What You Will Do
โ€ข Manages the enrollment process of prospective families and educates them on qualifications as it relates to the consumer, caregiver, and home; assesses consumer and caregiver eligibility for services
โ€ข Helps guide consumers with applications for Medicaid, waiver programs, and other state benefits
โ€ข Conducts initial in-person clinical assessments and coordinates service activation, ensuring that all required forms and standard procedures are followed
โ€ข Assists with orientation training for each caregiver to facilitate a proper onboarding to Careforth's HIPAA secure technology platform; ensures caregiver understands how to successfully submit electronic daily communications
โ€ข Works collaboratively to develop each member's plan of care and review on an ongoing basis
โ€ข Provide ongoing coaching to drive effective utilization of established best practices, encourage caregiver education, and connect families to community resources
โ€ข Reviews the caregiver notes daily and appropriately addresses changes in the consumer's condition or environment
โ€ข Completes documentation corresponding with each on-site visit or encounter
โ€ข Monitors the health status of all members to ensure that all needs and programmatic requirements are properly delivered (i.e. incident management)
โ€ข Collaborates and communicates regularly with clinical team, agency and support services, primary care physician, and other medical providers to ensure timely and appropriate delivery of consumer services
โ€ข Ensures continuous compliance with all regulatory and accreditation agency standards as well as company policies and procedures
โ€ข Plans and implements discharges and transitions from the program
โ€ข Perform other duties as assigned
What You Will Bring
โ€ข Must hold an active Registered Nurse (RN) License and/or be a Licensed Social Worker (LSW/LISW) in the state of Ohio
โ€ข Minimum two (2) years of recent clinical experience in direct care of elders or adults with disabilities
โ€ข Ability to travel throughout the state of Ohio as assigned, including potential overnight stays; Must possess a valid driver's license and maintain active automobile insurance.
โ€ข Experience in case management, care planning, and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
โ€ข Ability to work in a fast-paced environment meeting department goals and KPIs
โ€ข Ability to influence and be self-motivated with a results-driven approach
โ€ข Tech savvy with the ability to teach others; proficient in common software and web-based applications (e.g. Case Management Systems, MS Office)

The pay range for this position is $61,248- $84,200. To recognize the flexibility and travel associated with this role, the position includes a $500 biweekly travel differential in addition to base pay. All required travel expenses will also be reimbursed in accordance with the company policy. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
