Patient Care Coordinator – Orthopedics & Hand Surgery Position Summary

Are you someone who enjoys helping people, solving problems, and keeping things organized? We're looking for a Patient Care Coordinator to partner directly with one of our orthopedic/hand surgeons and play a vital role in delivering an exceptional patient experience.

As the primary clinical and administrative support for your assigned physician, you'll guide patients through every stage of their care journey-from office visits and treatment planning to surgery, recovery, therapy, and return to work. You'll collaborate closely with patients, providers, therapy staff, surgery schedulers, front desk team members, hospitals, and outside healthcare facilities to ensure care is coordinated, efficient, and patient-focused.

The ideal candidate is highly organized, customer-service oriented, and committed to delivering exceptional patient-centered care. This role is perfect for someone who thrives in a fast-paced environment, enjoys building relationships, and takes pride in helping patients navigate their treatment successfully.

What You'll Do
  • Serve as the primary Patient Care Coordinator for an assigned orthopedic/hand surgeon.
  • Educate patients and answer questions regarding treatment plans, surgery, post-operative care, medications, and recovery expectations.
  • Room patients, complete intake documentation, and assist with clinical patient flow.
  • Remove sutures and staples, unwrap wounds, and assist with post-operative visits as directed.
  • Coordinate communication between patients, physicians, therapy services, surgery centers, hospitals, and other healthcare facilities.
  • Schedule outside testing, imaging, and specialty appointments.
  • Manage patient forms, including disability, return-to-work, BWC, and other physician-completed documentation.
  • Resolve patient concerns, medication refill requests, and care coordination issues in a professional and timely manner.
  • Obtain insurance authorizations for diagnostic testing and other procedures
  • Manage gel injection and Xiaflex pre-certifications and scheduling.
  • Handle patient calls, portal messages, and voicemail requests promptly and professionally.
  • Experience with Electronic Health Records (EHR) required; NextGen experience strongly preferred. Ability to accurately document patient information, manage clinical workflows, and navigate multiple systems efficiently
  • Assist with special projects and additional duties as assigned.
