Job description

What You Should Know About This Role
  • Full-time position with a schedule of 8:00 AM – 4:30 PM.
  • Weekend rotation required approximately every 4–6 weeks.
  • This is a driving position serving patients throughout the Lancaster and Zanesville areas, with the majority of visits taking place in Zanesville.
  • Reliable transportation, a valid driver's license, and the ability to travel in varying weather conditions are required.
About the Role

As a Personal Care Specialist, you'll provide compassionate, hands-on care that helps patients remain comfortable and supported throughout their hospice journey. More than providing personal care, you'll build meaningful relationships with patients and families while serving as an important member of the interdisciplinary care team.

At Ohio's Hospice, we use the title Personal Care Specialist because we recognize the vital role our CNAs play in delivering care with dignity, compassion, and respect.

Responsibilities
  • Assist patients with personal care activities while preserving comfort, dignity, and independence.
  • Support family members by providing education and assistance with daily care needs.
  • Observe and report changes in patient condition to the clinical team.
  • Collaborate with nurses and other hospice professionals to help deliver individualized patient care.
  • Accurately document care and services in the electronic medical record.
  • Maintain professional, compassionate communication with patients, families, and team members.
Qualifications
  • High school diploma or GED required.
  • Active Ohio State Tested Nurse Aide (STNA/CNA) certification in good standing preferred.
  • One year of healthcare or patient care experience preferred.
  • Hospice and Palliative Nursing Assistant (CHPNA) certification preferred.
  • Basic computer skills for electronic documentation.
  • Valid Ohio driver's license with a safe driving record.
  • Current automobile insurance meeting state minimum requirements.
  • Ability to travel throughout the service area, including during evenings, adverse weather, or other driving conditions as needed.
