We are a well-established company serving multi-city areas in seven parishes. It is the

goal of this company to provide the best possible healthcare to

each and every client we serve.

A DSW (direct support worker) is an unlicensed individual employed by an agency to perform personal care services to clients in need, whether that be in-home, hospital, nursing facility

assisted living facility or similar.

EDUCATION:

High School Diploma or equivalent required.

Any other medical or in-home healthcare training preferred or DSW on-the-job experience.

Certifications, such as CPR & First Aid a plus.

QUALIFICATIONS:

Must be reliable, dependable and on time!

Outgoing personality with a positive attitude.

Detail oriented, able to multi-task.

Good at documenting daily requirements &/or reports.

Adapt quickly to change.

Work well with others, various personality types.

Respect clients, their families, visitors, company staff, supervisors & management.

Must be able to handle situations where a client or family is upset or has concerns.

DSW’S have access to confidential information, therefore you will be expected to sign a

confidentiality agreement that also states that at no time are you allowed to solicit

our clients or employees.

JOB FUNCTIONS/RESPONSIBILITIES:

Number one goal: Provide the best care possible when serving our clients per the ISP

(individual support plan)

You may be providing services to a client in-home or at a medical facility.

Personal care services include, but not limited to:

· Bathing

· Dressing

· Grooming

· Feeding or assistance as needed.

· Exercising

· Toileting

· Positioning

· Assistance with self-administered medications

· Routine hair & skin care

· Transfer or ambulation

Provide direct care services to the client at the time & location assigned

Report changes in client’s condition to a supervisor immediately upon discovery of change

Examples of incidents that must be reported immediately:

· Falls

· Burns

· Skin breakdown or wounds

· Pain

· Change in mental status

· Report a client’s request for services immediately

· Report a client’s request for change in services immediately

· Follow emergency medical training while attending to the client

· Report any other types of emergencies to the supervisor immediately

· Report any suspected abuse, neglect or exploitation of client to supervisor immediately, as required by law.

· Follow all required protocol to remain in compliance with the State of Louisiana

regulations and guidelines. (referred to state regs)

· Will document their activities daily in the client’s in-home binder.

· All required documentation must be turned into the office on time.

· Other duties as assigned by Supervisor.

· Any duties assigned by Executive Director based on new regulations from State of Louisiana to remain in compliance.
