Hyve Mental and Counseling Behavioral Center is seeking a dedicated and compassionate Licensed Mental Health Therapist (LCSW, LMFT, LPCC) to join our team in Visalia, California. This full-time position offers the opportunity to work remotely while supporting our outpatient services aimed at providing exceptional mental health care to a diverse patient population. Key Responsibilities

• Conduct comprehensive mental health assessments that evaluate the interplay between psychological wellbeing and any existing physical health issues affecting our patients.
• Develop and implement individualized treatment plans tailored to each patient's needs, incorporating evidence-based practices for both psychiatric and co-occurring medical conditions.
• Facilitate individual and group therapy sessions, providing a supportive atmosphere that encourages patient growth and recovery.
• Work closely with medical professionals to coordinate care, ensuring that all aspects of a patient’s health are managed for optimal outcomes.
• Perform crisis interventions during mental health emergencies, providing immediate support and determining further care needs.
• Empower patients through education on the relationship between mental health and physical conditions, enhancing their ability to manage their overall health.
• Ensure meticulous documentation of patient progress and treatment plans in compliance with legal and ethical standards, utilizing electronic health record systems.
• Participate in interdisciplinary team meetings and case conferences to discuss treatment strategies and share insights that promote integrated care.
• Keep abreast of current research and best practices in mental health treatment and integrated care methodologies.

Patient Population / Client Focus

Our patients are diverse, dealing with a variety of issues including, but not limited to, substance use disorders, chronic pain, autoimmune conditions, and other mental health challenges. You will be instrumental in navigating the complexities of their care, supporting them as they strive for improved mental and physical health.

Qualifications

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Valid and current licensure as an LCSW, LPCC, or LMFT in the state of California.
• Strong critical thinking and analytical skills to effectively assess and tailor treatment plans for patients with co-occurring conditions.
• Superior interpersonal communication skills to build rapport and trust with patients, families, and colleagues.
• Demonstrated ability to maintain professional boundaries while acting ethically and ensuring patient confidentiality.
• Proficient in using computer applications and electronic health record (EHR) systems.
• A collaborative mindset and ability to work effectively within a multidisciplinary team.

Preferred Experience

• Experience in an outpatient or integrated care environment focused on co-occurring medical and mental health conditions.
• Familiarity with specific therapeutic modalities that support integrated care, such as cognitive-behavioral therapy (CBT) and motivational interviewing.
• Previous experience conducting telehealth sessions and utilizing technology effectively to engage with patients.
