About BHT

Better Health Together (BHT) has been leveraging experience, knowledge, and resources to coordinate action and address local and regional health inequities across Ferry, Stevens, Pend Oreille, Spokane, Lincoln and Adams counties and the Reservations of the Kalispel Tribe of Indians, the Spokane Tribe of Indians, and the Confederated Tribes of the Colville reservation since 2014.

About the Position

The CCHAdministration and Navigation Coordinator is a temporary position through July 31, 2027,designed toprovideregional surge capacityduring a period of significant health coverage and service-delivery change.The responsibilitiesare:

  • Supportpatientand community memberoutcomes through referralsintothe Community Care Hub,which may includeHRSN eligibility screenings and referrals.Case assignments may vary in location(including rural areaswithin the BHT regionof Adams, Ferry, Lincoln, Pend Orielle, Spokane, Stevens, and the Tribal reservations of the Colville Confederated Tribes, Spokane Tribe of Indians, Kalispel Tribe of Indians).Mileage reimbursement is included in employment.
  • Provide administrative, referral, and data support across the Community Care Hub, including at least monthlyProviderOneeligibility checks and related routine system tasks,maintainingand pullingdata across Hub tracking and case-management systems
  • Work closely withHealth Benefit Exchangeand partnering organizations,Basic Food education & outreach, and provide administrative support for the Navigator program,determiningeligibility for multiple programs, enrolling in HPF, problem-resolution, and multi-agency coordination.
  • Responsibilities will flex based on regional and organizationalneed, including Open Enrollment, federal healthcoverages, CCH operations, HRSN initiatives, rural outreach, emergencyresponse, and other emerging community priorities.


Essential Job Functions

Impactful Problem Solver + Highly Organized

  • Provide administrative, referral, and data support across the Community Care Hub
  • CompleteProviderOneeligibility checks and related routine system tasks
  • Enter, organize, andmaintaindata across Hub tracking and case-management systems
  • Pull standard reports and compile data for staff and partner requests
  • Maintain tracking tools and support documentation processes
  • SupportGetCare, Tier 2 HRSN, and other expanding Hub initiatives
  • Cross-trainacross CCH, Navigator, HRSN, referral coordination, and eligibility workflows to support operational continuity during increased demand or changing communityneed.
  • Respond to routine Navigator, eligibility, referral, and partner-support requests, escalating complex cases to senior staff whenappropriate.
  • Identifyrecurring barriers, workflow gaps, and system issues and communicate them to program leadership for resolution.
  • Reduce administrative burden on referral coordinators, Hub managers, and quality-improvement staff
  • Conduct outreachandpatient-centered support&referralsin hospitals, clinics, rural communities, andcommunity-based settings.
  • Supportwarm handoffs tocare coordination by collaborating with interdisciplinary teams (providers, nurses, case managers, social workers)on an as-needed basis;
  • Deliver culturally responsive and community-informedinformation to partners,providers, nurses, case managers, social workers, community-based workers, and other relevant stakeholders;
  • Adapt to varying workflows, care teams, and environments, including independent work in rural and community-basedsettings;
  • Ensuresreferrals aretimely,accurate, and complete in the client management system.

Effective Coordination + Communicator

The CCHAdministration and Navigation Coordinatorpositionrequires developing andmaintainingtrustedrelationships with internal and external partners to ensure program satisfaction.

  • Strong interpersonal skills with the ability to build trust across diverse populations, including rural communities;
  • Excellent communication skills, with the ability to translate health information into understandable terms;
  • Ability to navigate both formal healthcare systems and informal community networks
  • Strong organizational and time management skills, especially in a flexible, ad hoc work structure;
  • Critical thinking and problem-solving skills, particularly in low-resource or geographically isolated settings;
  • Ability to work independently in rural or field-based environments as well as collaboratively within teams;
  • Reliability and strong follow-through in short-term or intermittent assignments;
  • Highly competentand confidentwith technology, including being a point-of-contact for tech questions for their team andhashigh comfort usingnew technology;
  • Experience with handling/managing and reviewing sensitive documents,includingPersonal Health Information (PHI);
  • "Super User" skill level forCCHClient Management System andotherClient Managementsystemsfor program/project coordination(these systems can be trained for)

Qualifications

  • Experience with care coordination,preferredCertificateas a Community Health Workeror similar certification;
  • Experience with health coverage enrollment, eligibility screening, benefit navigation, referral coordination, or related community-based services preferred.
  • Able to prioritize and multi-task in a fast-paced work environment; consistently delivering on established schedules, guidelines, protocols, and deadlines;
  • Ability to distinguish routine requests from complexcasesrequiring escalation or advanced policy interpretation.
  • Familiarity with Washington Healthplanfinder,ProviderOne, Medicaid, Marketplace coverage, HRSN,or public benefits is preferred but may be trained.
  • Lived experience that fosters connection with the populations served;
  • Experience working inand familiarity withresourcesinrural or underserved communities;
  • Cultural humility and respect for diverse lived experiences and community contexts; Empathetic, compassionate, and nonjudgmental approach;


Compensation

  • $69,000-80,000
  • Health Benefit premiums fully paid by BHT for you and your dependents (Medical, Dental, Vision, STD, Life)
  • 401k Employer Contribution of 6% by BHT (no employee match required)
  • 6 Weeks of PTO and Sick Leave
