    Job description

    Rose Mountain Care Center, centrally located in New Brunswick, NJ at is looking for a Full Time license Social Services Director with experience to join our dedicated team of healthcare professionals.

    Requirements:

    1. Graduate of an accredited school in social work or related field; having a bachelor’s degree in social work or a bachelor’s degree in a human service field including but not limited to sociology,  special education, rehabilitation counseling, and psychology.  
    2. State certified by the Board of Social Work Examiners as CSW or LSW.

    3. EXPERIENCE WITH MDS 3.0 / PHQ-9 / BIMs

      Long-term care / sub-acute experience strongly preferred.

     

    POSITION SUMMARY:

    Performs social services based on the facility’s policies and procedures as regulated by the State and Federal laws.

    PART 1: JOB RESPONSIBILITIES AND STANDARDS

    I.   Performs assessments relate to:  

    1.  Psychosocial functioning

    2. Cognitive Status

    3. Adjustment needs

    II.  Complete documentation

    1.  Initial Social Service History within 14 days of admission.

    2. MDS 3.0, Completes CAA’s

    3. Completes IDCP on admission. 

    4. Updates care plan as needed.

    5. Annual progress notes, readmissions from hospital and significant change.

    6. Hospital updates are completed within 48 hours.

    7. Discharge planning.

    8. Facilitating resident/family groups.

    9. Contacting families on various issues.

    10. Assist with resident satisfaction surveys.

    III.  Knowledge of:

    1.  Demonstrates knowledge of Federal regulations.

    2. Demonstrates knowledge of DOH regulations.

    3. Demonstrates knowledge of OBRA regulations.

    4. Participates in interdisciplinary meetings, weekly and as needed.


       PART II: FACILITY-WIDE RESPONSIBILITIES & STANDARDS

    A.   Demonstrates courtesy to residents, co-workers, and their families

    B.   Maintains resident confidentiality

    C.   Demonstrates ability work with others

    D.   Demonstrates flexibility in assignment to fill resident/family needs.


       PART III: PHYSICAL REQUIREMENTS OF THE JOB 

    1. Demonstrate ability to handle stressful situations.

    2. Must be able to write & speak the English language in an understandable manner.
    3. Must be able to see & hear or use prosthesis that will enable these senses to function.

    4. Must be able to see & hear or use prosthesis that will enable these senses to function.
    5. Must be able to sit, stand, bend, lift, push, and move intermittently during working hours.
    6. Ability to relate and work with the physically and emotionally challenged resident.

    Core Responsibilities include but are not limited to:

    • Develop preliminary/comprehensive assessments for each resident.

    • Encourage the resident/family to participate in the development and review of resident’s plan of care.

    • Discharge planning

    • Develop a written plan of care for each resident that identifies the social problems as well as the needs of the resident and set goals to be accomplished for each problem/need identified.

    • Ensure that the social services components of the MDS assessment are completed and signed in an efficient manner.

    • Review nurse’s notes to determine if the care plan in place is being followed.

    • Ensure quality customer service.

    • Timely completion of resident assessments

    The social worker will work with residents by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with the National Association of Social Workers (NASW) Code of Ethics along with strict adherence to government regulations. The social worker is responsible for fostering a climate, policies and routines that enable residents to maximize their individuality, independence and dignity. This climate shall provide residents with the highest practical level of physical, mental and psychosocial well-being and quality of life.
