UnderthemanagementofthePACESoutheast MichiganCommunityLivingManager,the Community Living Associate(CLA)isresponsible forprovidinghigh-quality,person-centeredcareand supportto participantsresidingincommunity living,congregatehousing,andassisted living environments.Community LivingAssociates work aspart of a24-hour,7-day-a-week care team and areexpectedtoprovide compassionate, respectful, and professionalcarewhilepromoting participantdignity, independence, safety, andqualityof life.TheCommunity LivingAssociate collaboratescloselywithnursing, home care,centerstaff,transportation, socialwork, dietary, andleadershipteams toensurecontinuity ofcarewithexcellentparticipantoutcomes.

EssentialDuties and ResponsibilitiesPersonal CareAssistance:

  • Assistsparticipants with activities ofdailyliving(ADLs),including but not limited to:

  • Bathingandpersonal hygiene

  • Dressing and undressing

  • Groomingandoralcare

  • Toileting andIncontinencecare

  • Ambulationand mobilityassistance

  • Feedingassistanceas needed.

  • Medicationadministration

  • Providesrespectful, dignified, andindividualizedcareaccordingto eachparticipant's care plan.

  • Observesand reportschanges inparticipantcondition, behavior, orfunctional abilitiesto theappropriatesupervisor.

Transfersand Mobility Support

  • Safelyassistparticipants with transfers/mechanical devicesaccording to training and safety protocols.

  • Ensuresparticipant safety during all transfers, repositioning, and mobility activities.

Participant Care and Supervision

  • Providessupport to participants with varying levels of physical, cognitive, and behavioral needs.

  • Engagesparticipants in meaningful daily interactions and activities that enhance quality of life.

  • Maintain a safe, clean, and supportive living environment.

  • Monitorsparticipant well-being and promptly reportsconcerns, incidents, accidents, or emergencies to leadership

Meal Service and Support

  • Assistswith meal preparationand dining support.

  • Providessnacks and hydration throughout the day as directed by participant care plans.

Housekeeping and Laundry

  • Performsdaily housekeeping duties, includingbut not limited to:

  • Cleaning/vacuuming/moppingparticipant room.

  • Sanitizing bathrooms and high-touch services

  • Completing participant laundry according to established schedules and standards.

  • Changing bed linens

  • Maintaining cleanliness throughout the community living environment

Morning and Evening Participant Support

  • Assistsparticipantswith morning routines to prepare for attendance at the PACE Southeast Michigan Day Health Center.

  • Ensuresparticipants are dressed appropriately, groomed, and ready for the dayand transportation if applicable.

  • Uponparticipantreturn from the day center,assistswith evening routines, meals, personal care, medication administration(if applicable and trained to pass medication), and bedtime preparation.

Community Living Associates (CLA) are expected to:

  • Demonstratesreliability, accountability, and professionalism at all times.

  • Reportsto work ontime andpreparedforassignedshift.

  • Takesownership of assigned responsibilities and participant care tasks.

  • Treatsparticipants, families, coworkers, and visitors with dignity, respect, and compassion,

  • Maintainsconfidentiality and professionalism in all interactions.

  • Supportsa culture of teamwork, communication, and continuous improvement.

Knowledge,Skills,and Abilities

  • Completionof nursing assistant program with certificateof completion. Acts only within the scope of authority to practice.

  • Minimum oftwo(2) yearsofexperience workingwith seniors incongregatehousing,long-termcare,orhospital settings.

  • Showscompassion,empathy,and respectfor thephysical, spiritual,and well-beingofparticipants,coworkers, andvisitors.

  • Must be medically clearedofcommunicablediseasesand have allimmunizationsup todateprior toprovidingdirect care.

  • Mustbeable toperformdutiesinan environment with frequentinterruptions andchangesincare/tasks.

  • Must beable toorganizeandmaintainflexiblescheduling tocoordinatejobresponsibilities.

WorkingConditions

  • Workingconditions are variable.Withexposure tocommunicabledisease, due toparticipantenvironment.There isalsoexposureforchemicals such ascleaningsolutionsand medication.

  • Must have the ability to transfer andassistwith moving participants; frequent bending, walking, lifting, 40 pounds or more may be needed in the performance of job duties.

  • May berequiredto changeassignmentsbasedonchanging participant acuityand level ofcareneedsandcensusaccording tomanagement or theInterdisciplinaryteam.
