Job description

Overview

Join the Loretto Health & Rehabilitation as a Clinical Coordinator, RN!

Under the guidance of the Unit Nurse Manager, The Clinical Coordinator is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.

Salary: $84,323 - $94,702 Annually 

Schedule: Monday - Friday- 08:00am-4:30 PM

Responsibilities

Responsibilities include:

  • Oversee the delivery of Quality Patient Care
  • Compliance with NYSDOH and CMS regulations and facility policies and protocols
  • Orients, trains and mentors new employees
  • Provides health training to patients and their families
  • Assists in gathering and analyzing data for QAPI
  • Acts as primary liaison with medical providers
Qualifications
  • 1+ year experience in nursing preferred
  • Current RN NYS license
  • Established patient assessment skills
  • Knowledge of best-practice standards
  • Skilled to respond to emergent situations
  • Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes
  • Exceptional interpersonal and customer service skills
  • Proficiency in speaking, listening, reading and writing the English Language is required
