KeystoneCare is seeking a dedicated, compassionate, and driven Registered Nurse (RN) to join our growing Home Health team on a fulltime basis! If you're passionate about delivering highquality, patientcentered care-and want the freedom to make a meaningful impact in the comfort of patients' homes-this is the role for you.

As a Home Health RN, you will assess patient needs, develop personalized care plans, and provide exceptional handson nursing care while following established clinical standards and agency policies. You'll collaborate closely with physicians, interdisciplinary team members, and families to ensure smooth care coordination and promote every patient's independence, safety, and wellbeing.

Primary Job Responsibilities:

  • Conduct comprehensive patient assessments to evaluate their healthcare needs.
  • Develop, implement, and update individualized care plans based on patient assessments and preferences.
  • Administer medications and treatments as prescribed by the physician.
  • Monitor and document patient progress and responses to treatment.
  • Educate patients and their families on health management, medication administration, and care plans.
  • Coordinate care with interdisciplinary team members to promote optimal health outcomes.
  • Ensure compliance with agency policies, state, and federal regulations.
  • Act as a patient advocate, promoting their rights and preferences in care.

Requirements

  • Current RN license in good standing to practice in the Commonwealth of Pennsylvania.
  • Graduate of an accredited nursing school (BSN preferred).
  • At least 1 year of nursing experience; experience in home health preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and manage time effectively.
  • Reliable transportation and valid Driver's License.
  • Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
  • Proof of Annual Influenza vaccination or an approved Medical or Religious exemption.
