Position Summary:

Fortune's CareManagement Unit (CMU) provides health care coordination services to highutilization Medicaid clients with criminal justice histories who have chronichealth and mental health conditions. We're currently seeking a dynamic,experienced Clinical Director to provide clinical and managerial oversight ofour rapidly growing Care Management Unit (CMU). Oversight of the unit will beinterdisciplinary in nature, incorporating medical behavioral expertise for thepurpose of providing comprehensive support and guidance of care managementactivities. S/he will work closely with his/her team to ensure consistentprovision of high quality face-to-face and telephonic outreach, assessment,intervention, referral, linkage, monitoring and service planning for allclients served. The Director will be responsible for ensuring that theindividual Health Home policies and procedures that guide the CMU's work areunderstood and adhered to by the members of the unit. S/he will compile andmaintain programmatic data reports and build strong working relationships withHealth Home networks, medical and behavioral health providers, social serviceproviders, and other stakeholders to align services that promote access to careand enhanced health outcomes for all clients. The Director will also beresponsible for ensuring an environment of trust and client well-being withinthe unit at all times.

Essential Duties and Responsibilities:

Providesclinical and operational supervision to the Care Management team, including Supervisors,Care Managers, Care Navigators and administrative personnel;

Monitorsperformance of team members in accordance with the policies and procedures oftheir assigned Health Home and provides training and coaching asnecessary;

Worksclosely with the agency's compliance unit to ensure audit-ready unitperformance at all times;

Utilizesmanagement tools and databases to track staff workload and activities;

Developsand implements performance improvement plans and manages a progressive disciplinaryprocess as needed; oversees the delivery of performance improvement plansacross supervisory team;

Assistsin the planning and implementation of operational procedures and providescontinuous feedback about operations; ensures all departmental policies are inadherence with regulatory bodies and Health Homes;

Servesas a point of contact for internal and external crisis intervention services;Writes and delivers performance evaluations; oversees the timely completion ofperformance evaluations for those under his/her supervision;

Collaborateswith referral sources including clinical care providers and otherentities;

Utilizesmultiple electronic health record systems and ensures that supervisory teamdevelops expertise in the EHR of the assigned Health Home(s); Promotesthe mission of the Fortune Society; and

Performother duties as assigned.

Supervisory Responsibilities:

Oversees CareManagement Team, under the guidance of the senior director. Carries out supervisory responsibilities inaccordance with the organization's policies and applicable laws. Supervisoryresponsibilities include training employees; planning, assigning, and directingworkloads; appraising performance; rewarding and engaging employees; addressingemployee relations and resolving problems.

Qualifications:

LicensedClinical Social Worker (LCSW), License Masters of Social Worker (LMSW), or aLicensed Mental Health Counselor (LMHC) is required;

Twoyears of post-Masters Degree experience in the emotional/behavioral assessmentand treatment of individuals in a mental health setting, including experiencewith those with substance use and/or chronic illness; working knowledge of HIVinfection;

Aminimum of two years managerial experience supervising staff who deliverservices to individuals living with chronic medical and/or severe andpersistent behavioral health needs;

Excellentinterpersonal skills with ability to interact culturally, linguistically anddiplomatically with diverse internal and external audiences; ability tomotivate, develop, and coach individuals and groups;

Strongorganizational skills to manage multiple priorities in a time sensitive manner;

Provenability to drive programmatic outcomes to success;

Proficientin Microsoft Excel, Word and Outlook; working knowledge of MAPP, ePACES,PSYCHES a plus;

Excellentwritten and verbal communication skills;

Bilingualskills (English/Spanish) a plus; and

Weseek talented, dedicated individuals from all walks of life who possess astrong commitment to this mission.Relevant personal experience is a plus.

Travel Requirements - travel to our Harlem office for meetingsand/or events.
