    Job description

    Hourly Pay Range:

    $38.67 - $59.94 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

    Position Highlights:

    • Position: Physical Therapist II

    • Location: 303 W Lake Street, Addison, IL

    • Full-Time

    • Clinic Hours: Monday through Thursday 7am to 7pm, Friday 7am to 5pm. Will work one evening until 7:00pm.

    At Endeavor Health, our mission is to put patient outcomes first and serve both our organization and the community.We currently schedule patients one on one for 45 minutes, with efforts made to keep patients with the same provider from start to finish.We also offer competitive internal education, to help you on your path to continual growth and keep mandatory requirements easy to maintain.For new grads, and those with less than three years' experience, we offer mentorship to ensure you have a great start to your career. We accomplish this by utilizing our many specialized therapists who teach at local universities. They provide mentorship within our residency and fellowship programs. While our primary population of patients is orthopedic physical therapy, we also offer Occupational and Speech services. We offer specialties in hand, neuro, vestibular, pelvic health, pediatrics, and lymphedema.

    What you will do:

    Provides direct physical therapy services to orthopedic patients.Demonstrates an appropriate level of clinical skill, as evidenced by the completion of comprehensive and insightful evaluations, and the development of individualized and effective treatment plans designed to achieve precise, functional goals.Evaluates patient's physical and functional status and plans and implements therapeutic programs that include prevention, identification, correction and alleviation of acute or prolonged movement dysfunction of an anatomic or physiologic origin. Uses excellent communication skills to educate patients and engage them in their own care.Stays updated regarding developments, new techniques, and equipment in the field of physical therapy.Maintains a well-rounded knowledge base, and clinical reasoning skills to consistently achieve expected clinical outcomes.

    What you will need:

    • Bachelor's Degree in Physical Therapy (if graduated before 2002) OR Master of Science or Doctorate in Physical Therapy (if graduated 2002 - 2014) OR Doctorate in Physical Therapy (if graduated 2015 and after).

    • Current State of Illinois Physical Therapist licensure

    • Current CPR Certification issued by the American Heart Association BLS provider

    • A valid driver's license is required if the incumbent is selected to perform related duties at an off site location. If the incumbent uses their personal vehicle, the incumbent must maintain automobile liability coverage as required by law and evidence of such coverage may be requested.

    • A minimum of 1-3 years experience with demonstration of clinical independence and experience in pelvic health physical therapy skills, preferred.

    • Will consider training therapist with experience in general orthopedics and demonstrated strong interest in pelvic health

    • Support available for certification and development

    Required without License:

    • Notification from Continental Testing Services advising of receipt and approval of application for licensure (if first-time test taker).

    • "Authorization to Test" (ATT) letter from Federation of State Boards of Physical Therapy

    • Confirmation of Computer-Based Exam

    • State ofIllinois Physical Therapylicense within 90 days of hire

    • Current CPR Certification issued by the American Heart Association BLS provider
