Job description

The Case Manager position provides services for early childhood aged children who qualify for targeted case management services due to delays in development or risk factors associated with developmental disabilities, or mental illness and whose families demonstrate a need for active case management services. Additionally, staff provide intensive case management, skill building, parent education, and support services to identified parents. Provides case management services in the home or other natural environment and coordinates services and resources needed by the families to support their development and enable them to remain and function optimally in the community. 

Salary Range:

$39,487.50- $45,410.63

Education/Experience Required: 

  • High school diploma.

Additional compensation will be considered for any new employees who speak multiple languages.
