Mobile Nurse Practitioner Opportunity

CareMore Health is seeking a Mobile Nurse Practitioner to provide comprehensive primary care services to patients in their homes and through virtual visits across San Gabriel Valley and East Los Angeles. Serving patients throughout San Gabriel Valley and East Los Angeles, you'll deliver comprehensive primary care in home-based and virtual settings while partnering with an interdisciplinary team committed to providing exceptional, compassionate care.

As a Mobile Nurse Practitioner, you'll bring high-quality primary care services directly to patients in their homes and virtually when appropriate. You will:

• Conduct comprehensive health assessments and follow-up visits
• Diagnose and manage acute and chronic health conditions
• Develop and implement personalized care plans
• Order, interpret, and evaluate diagnostic tests and laboratory results
• Prescribe medications and other treatments in accordance with state regulations
• Educate patients and caregivers on disease prevention and healthy lifestyle choices
• Collaborate with interdisciplinary care teams to coordinate care and improve outcomes
• Identify barriers to care and help connect patients with resources and support services
• Participate in on-call responsibilities as needed

Service Areas: This field-based role supports patients throughout San Gabriel Valley & East LA. Regular travel throughout the assigned territory is required.

What We're Looking For:

• Master of Science in Nursing (MSN)
• Active, unrestricted Registered Nurse (RN) license
• Active, unrestricted Nurse Practitioner (NP) license
• Minimum of one year of Nurse Practitioner clinical experience
• Strong assessment, diagnostic, and care management skills
• Valid driver's license and reliable transportation
• Ability to work independently and collaboratively within a multidisciplinary team
• Completion of required TB screening

• Primary care, family medicine, internal medicine, geriatrics, home-based care, or value-based care experience
• Experience caring for Medicare and medically complex patient populations
• Bilingual Spanish-English skills preferred

Compensation Range: $119,313.00 to $178,968.00
