Key Responsibilities:


- Act as an advocate for residents, ensuring their best interests are prioritized and protected.

- Assess the psychosocial status of residents in relation to their illness and environment.

- Communicate assessment findings effectively to the registered nurse and interdisciplinary team.

- Conduct social evaluations and develop person-centered intervention plans based on evaluation results.

- Maintain accurate and comprehensive clinical documentation in accordance with facility policies and regulatory requirements.

- Collaborate with residents, families, and healthcare team members to coordinate appropriate social services and support.

- Assist residents in accessing community resources, benefits, and other support systems.

- Ensure compliance with all background check requirements and facility policies prior to contact with residents.

Requirements:

Education

  • Bachelor’s or Master’s degree in social work.


Licenses/Certification

  • LCSW, LSW, or MSW license required.
  • Must have a valid driver’s license.


Experience

  • Experience in post-acute/ skilled nursing preferred.
