Title: Program Manager (75432)

Location: Oakland, CA (Remote - one day in a month from office)
- Manage clinical aspects of Primary Care Hybrid Payment Model from initiation through delivery and monitors for continuous improvement with focus on clinical quality
- Educate and work with external partners and participating practices to maximize performance on outcomes of the value-based payment models
- Responsible for planning, execution of initiatives to improve quality performance within specific value -based payment models
- Facility with data analysis and dashboards, including Excel and Blue Shield of CA web-based value-based payment platform
- Serve as a clinical expert with internal constituents to drive meaning HEDIS, Utilization, and member experience outcomes success and support external partners in creating actions plans to close care gaps
- Exercise independent judgment in developing methods, techniques and evaluation criterion for obtaining results.
Required Skills:
- Experience educating providers.
- Knowledge in Value-based payment models, HEDIS, or Medicare Stars
- Clinical workflow Optimization
Preferred Skills:

- Intermediate - Advanced Excel Skills
Education:
- Bachelor's degree or equivalent (RN, LVN, Medical Assistant)
- 2-3 years of healthcare experience required (Health Plan or Provider setting preferred)
