Competitive Pay
Annual earning potential of $100,000–$120,000+, depending on caseload and productivity.
Mileage reimbursement available.

What You'll Do
As a Home Health Physical Therapist, you'll:
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop individualized treatment plans to improve strength, balance, mobility, and independence.
- Provide therapeutic exercises, gait training, balance training, and fall prevention education.
- Educate patients and caregivers on home exercise programs and safe mobility techniques.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
- Complete accurate electronic documentation.
- Monitor patient progress and modify treatment plans as needed.
Qualifications
- Active Colorado Physical Therapist License
- Graduate of an accredited Physical Therapy program
- Excellent communication and patient education skills
- Ability to work independently
- Reliable transportation and valid driver's license
- Home Health experience preferred, but we're happy to train clinicians who are eager to transition into Home Health.
