Job description

Step into a meaningful role within our Hospice Care Team as a Program Manager for Advance Illness dedicated to supporting patients and families during some of life's most vulnerable moments.  You'll serve as a trusted resource for nurses, physicians, caregivers, and loved ones, helping maintain comfort, dignity, and continuity of care.

Job Overview:

This role is responsible for day-to-day management and performance of HOC Navigators Palliative Care programs (inpatient, ambulatory, home-based, assisted living/long term care) and Advance Care Planning program. Manages work processes including referral, admission, care, discharge, and transfer. Ensures collaboration and coordination across programs internal and external to HOC Navigators. Achieves pillar goals in areas of people/culture, quality/safety/compliance, service, growth, and finance. Leads in accordance with TriHealth Way of Leading, SERVE values, and ALWAYS behaviors. Reports to the Senior Manager Advanced Illness Programs. Serves as a member of the HOC Navigators management team.

Job Requirements:
Bachelor's Degree In nursing, healthcare, or related field. (Required) 
Master's Degree (Preferred)
3 - 4 years Healthcare management required. (Required) 
3 - 4 years Previous practice management or palliative care program experience preferred. (Preferred)
Other Center to Advance Palliative Care (CAPC) Required 
Other National Partnership for Healthcare Innovation (NPHI) Required 
Other Care Solutions Network of Ohio (CSNO) Required
Participates in regional and national palliative care professional organizations including: Care Solutions Network of Ohio, Center to Advance Palliative Care, and National Partnership for Hospice & Healthcare Innovation.
Job Responsibilities:
People/Culture: Creates a caring and compassionate culture and a high level of provider and team member engagement; Recruits, selects, onboards and develops team members to create a high-performing team; Promotes specialty certifications; Develops staffing plans and work schedules to maintain continuity, quality, and efficiency of care delivery. 
Quality/Safety/Compliance: Develops and Maintains work processes to support evidence-based, standard work and to promote patient & family care and service that is safe, timely, effective, efficient, equitable, and patient-centered; Engages team members in continuous performance improvement to ensure excellence in care delivery. Ensures accurate documentation and coding. Achieves operating and strategic goals. 
Service: Ensures a high level of satisfaction among patients, families, and referral partners by promoting responsive, coordinated, and compassionate service. Addresses service recovery issues in a timely manner and drives continuous improvement in patient and referral partner experience. 
Financial: Assists in the development and management of operating budgets; Achieves revenue, expense, operating margin and other budget targets; Monitors and manages productivity; Coordinates billing processes to support accurate and timely reimbursement. 
Growth: Achieves volume goals for each program or service; Assists HOC senior managers in piloting and implementing new programs and services; Supports population health and value-based initiatives that expand access, improve outcomes, and align with organizational growth priorities.
