    Job Summary: Outpatient Clinic Patient panel – Primary Care – Geriatrics Essential Duties and Responsibilities: Coordinates the multidisciplinary care delivered to their panel of patients, to optimize patient care. Reviews patient medical records to diagnose, treat, and provide care management and education to patients and families. Makes appropriate patient referrals. Champion a culture of empathy, dignity, and respect for aging populations. Monitors national health guidelines and those established by JPS Health Network and Acclaim Health for the management of patient care. Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior. Maintains a collaborative environment with other referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care. Supports academic, educational and research endeavors. Assess and manage chronic and acute conditions in patients with multiple comorbidities. Partners and communicates with acumen to build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Health. This would include health network staff, vendors, outside partners, patients, stakeholders, and local community leaders. Serves as a resource to the community. Performs other job-related duties as assigned.

    Qualifications: Required Qualifications: Internal Medicine or Family Medicine residency and fellowship in Geriatric Medicine completion from accredited ACGME programs. Board certified by ABFM or ABIM required. Will consider candidates who are working towards CAQ in Geriatric Medicine within 5 year of joining. Required Licensure/Certification/Specialized Training: Current, unrestricted medical license to practice in the State of Texas. Board Certification or Board Eligible. Unrestricted work authorization to work in the United States. Preferred Education/Licensure/Certification and Experience: Bilingual or multilingual proficiency is a plus. Prior experience in post-acute or long-term care settings strongly preferred. Proficient with electronic medical records and knowledgeable of CMS/SNF regulatory requirements. Key Competencies: Must be comfortable working in a large public/county facility Must be able to demonstrate a working knowledge in area assigned, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area Must have good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community Must be self-directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area Opportunity to supervise Advanced Practice Professionals.
