Job Summary:
The Family Connections Consultant provides direct, home-based support services to families involved with the Department of Child Safety (DCS). This role focuses on strengthening family protective factors, enhancing caregiver capacities, promoting child safety, and supporting family stability through culturally responsive, trauma-informed, and evidence-based interventions and support services.
Responsibilities:
Work directly with families involved with the Department of Child Safety to support child safety and family stability. Manage an assigned caseload and ensure timely completion of required services, documentation, referrals, and follow-up activities. Conduct home visits and community-based meetings to assess family needs, monitor progress, and provide supportive interventions. Conduct assessments to help ensure a safe environment for children.
Provide trauma-informed, family-centered interventions and support related to parenting, mental health, domestic violence, substance use, and other identified family needs. Meet with families to provide consultation, education, and skill development related to family functioning and safety. Support services that strengthen family protective factors and enhance caregiver capacities.
Assist with service planning, linkages, and transition to ongoing supports, including helping families access financial, social, medical, educational, and employment resources. Provide substance use education and linkage to treatment providers as needed. Monitor, observe, and document interactions and behaviors between parents/caregivers and children during home visits, including modeling appropriate behavior and de-escalating conflict.
Work cooperatively with the Department of Child Safety (DCS) to support assessments, service planning, required reports, and meetings. Develop caring, supportive, strength-based, culturally competent, and trauma-informed relationships with families and community partners. Complete accurate, timely, and thorough documentation, including assessments, progress notes, service plans, incident reports, activity logs, statistical reports, and other required records in accordance with program, contractual, funding, and regulatory requirements.
Establish and maintain confidential client records and case files. Accurately record and submit time worked in accordance with organizational policies and wage and hour requirements. Participate in an on-call rotation and respond to client or program needs in accordance with program requirements. Maintain professional representation of A New Leaf in all interactions with clients, staff, and community members. Attend meetings, trainings, and special projects as requested by management. Perform other reasonably related duties as assigned.
Qualifications:
Education:
Bachelor of Social Work or a related bachelor's degree required.
Experience:
Minimum of one (1) year of experience in the human services field. Experience working with multi-problem families and complex family systems. Knowledge of family dynamics and family-centered service approaches. Knowledge of community resources and ability to assist families in accessing services. Experience supporting families impacted by substance abuse, child abuse and neglect, trauma, sexual abuse, or related challenges.
Experience working with families involved in child welfare, family preservation, family reunification, or Department of Child Safety programs preferred. Working Conditions/Physical Requirements: Work is performed in office settings, community locations, client homes, and other environments where services are delivered.
Frequent local travel is required to conduct home visits, attend meetings, and provide community-based services. May be exposed to a variety of environmental conditions and unpredictable situations while working in client homes and community settings. Frequent use of computers, telephones, and standard office equipment. Ability to sit, stand, walk, bend, reach, and drive for extended periods throughout the workday. Occasional lifting and carrying of up to 25 pounds.
Ability to manage multiple priorities and deadlines in a fast-paced environment. May be required to work occasional evenings, weekends, or modified schedules to meet client and program needs. Compliance Requirements: Must Be At Least 21 Years of Age Central Registry Background Check Drug Screening CPR/First Aid Certification Valid Unrestricted Driver's License Current Auto Liability Insurance 39 Month Motor Vehicle Record Use of a personal vehicle to conduct agency business and transport clients, when required. Ability to obtain and maintain all required certifications, clearances, licenses, and insurance throughout employment. Adhere to all organizational policies, including the Code of Conduct, professional standards, and relevant regulations.
Mandated Reporter Requirements: As a mandated reporter under Arizona statute A.R.S. § 13-3620, you are required to immediately report any suspected abuse or neglect of a minor to your Supervisor, Manager, Director, Clinical Director, or Chief Officer, who will assist in making the appropriate report to authorities. Reports should be directed to either the local law enforcement agency or child protective services (1-888-SOS-CHILD). Abuse suspected of involving a family member or legal guardian should be reported to child protective services, while other cases should be reported to law enforcement.
