Yorktown Manor is a premier skilled nursing facility in Delaware County, IN, providing both short-term and long-term rehabilitation, memory care, and hospice services. Our mission is to transform rehabilitation and nursing care in the Yorktown community by delivering compassionate, innovative, and patient-centered care.
We offer a home-like environment with a wide variety of therapeutic programs, including physical, occupational, and speech therapy, pharmacogenetics-guided medication management, and specialty staff training.
Position Overview
We are seeking a Certified Occupational Therapy Assistant (COTA) to join our in-house rehab team. The ideal candidate will have prior experience in skilled nursing/long-term care settings and familiarity with the Net Health electronic documentation system. This full-time position offers 35 hours per week with a flexible schedule.
Responsibilities
  • Provide direct occupational therapy services under the supervision of a licensed OT.
  • Assist with patient evaluations and contribute to individualized care plans.
  • Deliver goal-driven treatment programs to help residents improve independence and functional abilities.
  • Maintain accurate and timely electronic documentation of therapy sessions.
  • Collaborate closely with therapists, nursing staff, and interdisciplinary teams to enhance resident outcomes.
Qualifications
  • Certified Occupational Therapy Assistant (COTA) license in good standing.
  • Prior experience in skilled nursing or long-term care preferred.
  • Experience with Net Health or other electronic documentation systems is a plus.
  • Strong communication, teamwork, and a resident-focused mindset.
