Position Overview

West Valley Health System PC is seeking a dedicated and compassionate Licensed Independent Clinical Social Worker (LICSW) to join our team in a full-time remote capacity. Key Responsibilities

• Conduct thorough psychosocial assessments to evaluate clients' mental health concerns, identifying their strengths and challenges.
• Facilitate individual, family, and group therapy sessions, tailoring therapeutic interventions based on each client's unique needs and cultural backgrounds.
• Utilize evidence-based methodologies, including cognitive-behavioral therapy (CBT), motivational interviewing, and family systems therapy, to foster coping and resilience.
• Develop and implement individualized treatment plans outlining specific goals, interventions, and mechanisms for progress monitoring.
• Collaborate seamlessly with a multidisciplinary team, including psychiatrists, nurses, and other healthcare professionals, to coordinate holistic care for clients.
• Offer crisis intervention services by conducting risk assessments and developing safety plans that address immediate concerns.
• Advocate for clients' rights, connect them with appropriate community resources, and provide essential case management support.
• Participate actively in community outreach initiatives that bolster mental health awareness and reduce stigma.

Patient Population / Client Focus

You will work primarily with clients facing significant emotional and psychological challenges related to end-of-life issues. This includes individuals who are dealing with terminal illness, bereavement, and the complexities of navigating grief. Your expertise will be instrumental in helping families find meaning, establish closure, and manage the inevitable changes that come with such life transitions.

Qualifications

• A Master’s degree in Social Work from an accredited program.
• Possession of an active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is required.
• Demonstrated strong communication, empathy, and problem-solving skills are essential for effective client interaction.
• Comprehensive knowledge of cultural competence and a commitment to working with diverse populations.

Preferred Experience

• Experience working in a community mental health setting or hospice care environment is strongly preferred.

Compensation and Benefits
