POSITION SUMMARY: The Peer Navigator will provide concrete services geared toward assisting men and women who have histories of medical, behavioral and mental health needs. ย The Peer Navigator will provide the peer support to engage the individual on a continuum of care to help the individual achieve and maintain motivation for continuing their care while maintaining recovery from substance abuse and mental illness.ย
POSITION DESCRIPTION: VIP Ryan White Wellness Center
Conduct in person outreach to clients having difficulty maintaining adherence to treatment ย ย
(HIV Care/Mental Health/Medical/Substance Abuse Treatments) ย
Outreach phone calls and letters as well as follow up ย
Accompany clients to appointmentsย
Make home or agency related visits ย
Promote pro-social behaviors ย ย
Facilitator groups (Wellness, Recovery Peer Support, & others as directed) ย
Participate in outreach events to represent the agency ย
HIPPA compliance ย
Attend all agency program staff meetings & trainingsย
Special Projects assigned by Supervisorย
Requirements
JOB COMPETENCY: ย
HS Diploma or equivalent preferredย
NYC Department of Health AIDS Institute Peer Certification preferred.ย
Be eligible to achieve NYC Department of Health AIDS Institute Peer Certification within 2 months from hiring.ย
Prior experience working with the HIV/Mental Health/Medical/Substance Abuse populationย
Excellent written and oral communication ย ย
Ability to speak, read and write the English languageย
Bilingual English/Spanish a plusย
