Position Overview

As the Weekend Admissions Liaison, you will serve as the primary point of contact for weekend admissions, ensuring a seamless and supportive experience for patients and their families. This role requires strong clinical insight and regulatory knowledge to support safe and compliant admissions.

Key Responsibilities

· Manage weekend inquiries and guide families through the admissions process

· Collaborate with nursing, rehabilitation, and administrative teams to confirm clinical eligibility

· Complete admission paperwork and collect all required documentation

· Ensure PASRR screenings are completed accurately and in a timely manner prior to admission

· Maintain clear and consistent communication with hospitals, families, and facility staff

· Represent the Vantage Care with professionalism, compassion, and positive energy

Qualifications

· Must be a licensed professional: RN, LPN, or Licensed Social Worker

· Experience in admissions within a healthcare or long-term care setting preferred

· Demonstrated knowledge of PASRR requirements and ability to complete screenings independently

· Strong communication skills with a customer-service-focused approach

· Highly organized with the ability to multitask and manage busy weekends

· Professional, approachable, and calm under pressure
