Key Responsibilities

• Conduct comprehensive evaluations, analyzing patient history to create individualized treatment plans that enhance mobility and overall well-being.
• Communicate evaluation results and treatment plans effectively to patients and their families, ensuring that their goals and concerns are integrated into the therapeutic process.
• Implement and adjust treatment protocols tailored to each patient's specific needs, consistently monitoring progress and adapting plans as the patient's condition evolves.
• Collaborate with a multidisciplinary team of healthcare professionals, documenting changes in patient status and adjusting care plans as necessary.
• Maintain accurate and timely documentation of all patient interactions, including evaluation summaries, treatment notes, and discharge plans, to ensure continuity of care.
• Manage time effectively to meet productivity expectations while maintaining flexibility and adaptability to address patient needs.
• Engage in hospital and community outreach initiatives to enhance patient access to care and promote department goals.
