We are currently seeking a PT PCP with at least 1 year of experience in family medicine.  You will focus on providing high quality patient care as part of an amazing healthcare team, that prides itself on Professionalism, Proficiency, Patient Rapport and Quality Care!
This is a Medical and Diagnostic practice with two locations in Queens (Glendale & Rego Park). At this time, CityChoice is looking to bring on a family medicine physician with outpatient experience. Responsibilities include Adult Routine Care, Geriatrics, Women’s and LGBTQ Healthcare. Outpatient experience preferred. Salary will commensurate with experience.  
EMR: Tebra (formerly Kareo)
Bilingual: Spanish

Responsibilities:
Assess, diagnose, and manage a wide range of acute and chronic illnesses, such as high cholesterol, COPD,  diabetes, hypertension, infections, and injuries.
 
Order and analyze laboratory and diagnostic tests, such as blood work, ultrasounds and X-rays, to evaluate a patient's condition and guide treatment.
Annual Physicals, Bronchitis, Cancer, Diabetes, HTN,  Immunizations, Strep Testing, STD, Pap Smear, Respiratory, Wound Care, Suture Removal, Diagnostic Referrals, Medical Clearance, Controlled Substances, etc 
Perform medical procedures as necessary
Prescribe medications and treatments
Assess patient health through interviews and physical examinations
Communicate information to patients regarding diagnosis and treatment goals
Maintain a good doctor-patient rapport
Facilitate referrals to other healthcare professionals and medical facilities
Qualifications:
In-Network with Major Insurances (Preferable)
Advanced knowledge of treatment and drug interactions
Medicare/Medicaid Number
Malpractice
NY Medical License
Strong Leadership Qualities
Board Certified
