Licensed Vocational Nurse (LVN) — Field Nurse, Riverside County


Service Area: Riverside County
Setting: Recuperative Care / Short-Term Post-Hospitalization / Community-Based Care

Position Overview

UCanFeelBetter is seeking a dependable, highly responsive Licensed Vocational Nurse to provide mobile nursing support to members residing in our Riverside County homes.

This is a field-based position. The LVN will travel between assigned UCFB locations to complete routine nursing visits and respond to same-day/immediate clinical visit requests when needed.

The LVN will work under the direction of UCFB’s RN Medical Department Lead and will serve as the Medical Department’s eyes and ears in the field.

Primary Responsibilities

  • Visit assigned Riverside County homes on a routine/daily basis based on member needs and RN direction.
  • Respond to same-day nursing visit requests when clinically appropriate and assigned.
  • Obtain and document vital signs.
  • Perform nursing assessments and observations within LVN scope.
  • Assist with medication reconciliation and medication compliance monitoring.
  • Identify medication discrepancies and report them to the RN.
  • Review medications with members and help identify refill or pharmacy needs.
  • Monitor blood glucose/Accu-Chek information when applicable and within scope.
  • Assess and document changes in member condition.
  • Assist with nursing intake requirements for new members.
  • Complete required nursing forms and clinical documentation.
  • Document every member encounter accurately and promptly.
  • Communicate abnormal findings immediately to the RN Medical Department Lead.
  • Assist with DME and medical follow-up needs.
  • Communicate member concerns involving medications, appointments, wounds, falls, mobility, respiratory status, glucose, or other medical issues.
  • Coordinate with house staff and case management when appropriate.
  • Follow established escalation procedures for urgent or emergent conditions.
  • Submit a clear daily report of completed visits, findings, outstanding needs, and required follow-up.

Daily Expectations

The LVN should begin each day knowing:

  • Which homes require visits.
  • Which members require assessment.
  • Which new admissions need nursing attention.
  • Which members require follow-up from previous visits.
  • Which immediate requests have been assigned by the RN.

Before the end of the day, all visits should be documented and unresolved clinical concerns communicated to the Medical Department Lead.

Requirements

  • Active California LVN license in good standing.
  • Valid driver’s license.
  • Reliable transportation.
  • Ability and willingness to travel throughout Riverside County.
  • Strong clinical documentation skills.
  • Ability to use a laptop/electronic documentation system in the field.
  • Strong communication and time-management skills.
  • Ability to work independently while following RN direction.
  • Dependable and responsive to same-day requests.
  • Comfortable working with medically and socially complex populations.

Experience in home health, recuperative care, SNF, post-acute care, behavioral health, community health, or Medi-Cal populations is strongly preferred.

Ideal Candidate

We want someone who pays attention, notices changes, documents thoroughly, communicates quickly, and makes sure concerns reach the RN before they become bigger problems.

Reliability, communication, documentation, and clinical awareness are essential to this position.
