Key Responsibilities

• Conduct in-depth assessments of patients to identify their emotional, social, and psychological needs.
• Develop individualized treatment plans that address unique patient circumstances and goals.
• Utilize evidence-based therapeutic interventions, including mindfulness, cognitive behavioral therapy, and narrative therapy, to facilitate healing and coping.
• Document all patient interactions and treatment process meticulously to ensure confidentiality and compliance with regulations.
• Collaborate with a multidisciplinary team, including physicians, nurses, and other healthcare providers, to coordinate comprehensive patient care.
• Regularly review and update treatment plans based on patient progress and feedback.
• Engage in continuous professional development by staying current on research and best practices in the field of social work and hospice care.

Patient Population / Client Focus

You will primarily work with patients in a hospice center who are navigating the challenges of terminal illnesses, such as cancer, heart disease, and neurological disorders. Your role involves providing emotional support to patients and their family members, facilitating effective coping mechanisms, and helping them to navigate end-of-life issues with dignity. You will also address the profound emotional impacts of grief and loss, utilizing your skills to foster resilience in families during challenging times.

Qualifications

• A Master's degree in Social Work (MSW) from an accredited institution.
• An active Licensed Clinical Social Worker (LCSW) license in the state of Virginia.
• Demonstrated ability to communicate effectively with clients and interdisciplinary team members.
• Strong problem-solving skills and a commitment to ethical practice.

Preferred Experience

Compensation and Benefits
