Job description

Description:Trio Home Health is seeking compassionate and dedicated Registered Nurses (RN) to join our team. We are looking for RNs who are passionate about providing quality home health to our community-based patients in Denton, TX and the surrounding areas.

Requirements:

Registered Nurse (RN) Responsibilities:


  • Follow a designated care plan in accordance with patient's needs
  • Make home visits to clients in designated geographic territories
  • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
  • Monitor clients' conditions, reporting changes to Clinical or Client Services Manager
  • Follow up with, execute, and properly document doctors' orders
  • Perform client assessments
  • Case management and coordination
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications:


  • Current State or Compact Registered Nurse (RN) License in hand
  • A minimum of one-year clinical experience is preferred
  • OASIS knowledge highly preferred for home health applicants
  • Demonstrate knowledge and skillset in current nursing practice
  • Possess a valid state driver's license and automobile liability insurance
  • HCHB EMR knowledge is desired
