Job description

The Family Nurse Practitioner (FNP) will provide comprehensive healthcare services within a group practice setting, focusing primarily on adult patients. This role requires frequent travel between facilities to ensure continuous and effective patient care across locations. The FNP will perform thorough assessments, develop treatment plans, and deliver health education to promote wellness and manage chronic conditions.

Responsibilities

  • Conduct patient assessments to evaluate health status
  • Develop and implement treatment plans tailored to individual patient needs
  • Prescribe medications in accordance with clinical guidelines
  • Provide health education to patients to support disease prevention and management
  • Manage patient follow-up to monitor progress and adjust care plans
  • Maintain accurate and timely documentation of patient interactions and care

Preferred Qualifications

  • Minimum of 3 years experience in family nursing
  • Master's Degree in Nursing or related field
  • Certification as a Family Nurse Practitioner (FNP)
  • Proficient in care planning and coordination
