Job description

The Nurse Practitioner (NP) provides telehealth-based clinical services focused on pharmacogenomic (PGx) medication reviews, medication adherence initiatives, provider collaboration, and bridge prescription support. The NP serves as a clinical resource to members and prescribing providers by reviewing medication-related information, communicating clinically relevant findings, coordinating care, and supporting continuity of treatment.


This role does not involve establishing new diagnoses or independently developing treatment plans. The NP works within established clinical protocols and collaborates closely with prescribing physicians, primary care providers (PCPs), pharmacies, and internal clinical teams.


Essential Functions


Telehealth PGx Review Services

  • Conduct scheduled telehealth visits with members who have completed pharmacogenomic (PGx) testing.
  • Review PGx results and current medication regimens prior to member appointments.
  • Evaluate potential gene-drug interactions, medication sensitivities, therapeutic concerns, and other clinically relevant findings.
  • Explain PGx findings and the purpose and limitations of pharmacogenomic testing in a clear, patient-friendly manner.
  • Identify findings that warrant discussion or follow-up with the member's prescribing provider.
  • Document encounters and prepare concise clinical summaries and recommendations for the member's PCP or treating provider.
  • Participate in quality assurance activities related to clinical documentation and telehealth services.


Bridge Fill Support Services

  • Review patient data to identify members who may qualify for bridge medication fills.
  • Assess medication adherence indicators for diabetes medications, statins, RAS inhibitors, and other targeted therapies.
  • Compile clinical documentation required to support bridge-fill requests.
  • Communicate with provider offices to obtain necessary clinical information.
  • Follow approved outreach, workflow, and escalation protocols.
  • Coordinate with prescribing physicians regarding bridge prescription eligibility.
  • Submit bridge-fill prescriptions on behalf of authorized prescribing physicians in accordance with applicable regulations, clinical protocols, and organizational policies.
  • Document bridge-fill activities accurately within required systems.


Provider Collaboration & Care Coordination

  • Serve as a clinical liaison among members, prescribing providers, pharmacies, and internal care teams.
  • Support medication optimization and adherence initiatives through clinical review and provider communication.
  • Facilitate referrals and follow-up recommendations to treating providers.
  • Collaborate with care management, pharmacy, quality, and population health teams.
  • Escalate urgent clinical concerns according to established protocols.
  • Maintain professional and collaborative relationships with provider offices and external stakeholders.


Compliance & Quality

  • Maintain compliance with applicable state licensure requirements, HIPAA, privacy regulations, and organizational policies.
  • Follow established clinical protocols, standing orders, and workflow procedures.
  • Maintain accurate, timely, and complete clinical documentation.
  • Participate in ongoing clinical training, calibration sessions, and quality reviews.

Physical Requirements / Work Environment

  • Remote position with occasional travel of less than 5%.
  • Prolonged periods of sitting at a desk and working on a computer.
  • Ability to speak, hear, read, and comprehend written and verbal communications.
  • Must maintain a home office, mobile phone, and computer that meet organizational security requirements.
  • Minimum internet speed of 100 Mbps download and 10–20 Mbps upload.
  • Minimum typing/data-entry speed of 30 WPM.


Required Qualifications

  • Current, unrestricted Nurse Practitioner (APRN/NP) license in New Jersey and Georgia.
  • National board certification as a Nurse Practitioner.
  • Minimum of two (2) years of clinical experience as a Nurse Practitioner.
  • Experience with medication management, medication reconciliation, and chronic disease care.
  • Strong clinical assessment, critical thinking, and decision-making skills.
  • Strong clinical documentation skills.
  • Experience using electronic health records (EHRs) and telehealth technology.
  • Excellent patient education, communication, organizational, and interpersonal skills.


Preferred Qualifications

  • Experience in telehealth or virtual care.
  • Experience in pharmacogenomics (PGx), precision medicine, or medication therapy management.
  • Experience in population health, care management, managed care, or value-based care programs.
  • Experience supporting Medicare, Medicaid, Dual Eligible, Commercial, or Special Needs populations.
  • Familiarity with medication adherence programs, HEDIS, STAR Ratings, NCQA standards, or other healthcare quality initiatives.
  • Prior experience supporting bridge prescription programs.
  • Multi-state NP licensure.
