Job Summary

The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Job Requirements

• Current Florida license as a Physical Therapist
• Current CPR certification
• Preference for at least one (1) year of experience working in a Skilled Nursing Facility

Essential Duties & Responsibilities

• Creates, implements, and executes patient treatment plans
• Communicates with patients, families, referring physicians, and other members of the team
• Provides timely written documentation per facility, department, and insurance provider requirements to meet licensing, certification, regulatory, and reimbursement requirements
• Develops and enhances clinical and professional skills through knowledge and professional associations
• Supports and participates in department operations and development
• Reports any changes in patient status or needs to the Director of Rehab and interdisciplinary team as appropriate
• Supervises Physical Therapy Assistants to ensure optimal patient care and achievement of outcomes
