Job description

Nurse Practitioner (NP) / Physician Assistant (PA) – Post-Acute Care

Texas Post Acute Care is expanding and seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing clinical team. This position offers the independence and autonomy of private practice with the support of a collaborative healthcare environment.

The provider will care for patients at partner post-acute and skilled nursing facilities throughout Northeast San Antonio and New Braunfels, TX.

Responsibilities:

  • Perform comprehensive patient assessments, histories, and physical examinations
  • Diagnose and manage acute and chronic medical conditions
  • Develop and implement evidence-based treatment plans
  • Prescribe medications and monitor patient response and progress
  • Order and review appropriate laboratory testing and diagnostic studies
  • Educate patients and families regarding diagnoses, medications, and treatment plans
  • Collaborate with physicians, nursing staff, therapists, and other members of the interdisciplinary care team
  • Assist with transitions of care and discharge planning
  • Maintain timely and accurate EHR documentation
  • Travel daily to assigned partner facilities within the designated service area

Qualifications:

  • Active Texas Nurse Practitioner (NP) or Physician Assistant (PA) license
  • Master’s degree from an accredited NP or PA program
  • Current board certification
  • DEA registration or ability to obtain
  • Strong clinical judgment, communication, and organizational skills
  • Ability to work independently while collaborating with a healthcare team
  • Reliable transportation and ability to travel locally between assigned facilities

Post-acute care, skilled nursing, internal medicine, family medicine, hospitalist, or geriatric experience is preferred but not required.

Pay: $120,000 per year
