TRA Group, Inc. is a fast-growing nonprofit provider of volunteer medical transportation and community resource navigation services that help older adults and adults with disabilities remain independent, connected, and safely aging in place in their communities. Working out of their home offices, our Aging & Disabilities Program Coordinators ensure smooth daily operation of the TRA Call Center, linking older adults and adults with low vision with rides to medical appointments in Westchester, Putnam, and southwest Fairfield County, CT. When TRA volunteers cannot provide the rides, coordinators refer to other organizations and services. TRA coordinators also provide information and referrals to services for older adults and adults with disabilities throughout the lower/mid-Hudson Valley region. As TRA expands these services, coordinators will play an increasing role in community resource navigation and referral support. Experience in aging services, disability services, social services, care coordination, case management, or community resource navigation is highly desirable.
Essential Duties and Responsibilities:
Communications
• Ensure that callers feel welcomed and appreciated when they call the office, with the goal of sustaining their interest and enthusiasm in the program. For example, answer the phone within agreed-upon number of rings and returns calls within a desired number of minutes
• Be available for troubleshooting throughout assigned times
• Provide information and referral regarding transportation, caregiver support, housing resources, home care, vision services, nutrition programs, social engagement opportunities, and other services that support independent living
• Communicate regularly with referral partners on the TRA Resource Hub
Database and Trip Management
• Monitor the volunteer management platform to ensure that upcoming trips are covered and that trip details are relayed to volunteers and riders
• Ensure that ride requests are filled and confirmed at least one day before the ride
• Assign TRA vehicles & backup livery services when there is no volunteer to cover rides
General and reporting
• Provide thoughtful and on-going recommendations to improve and expand the program
• Implement change in procedures as updated and approved by the Director of the Call Center
• Assist in other program areas when necessary
Qualifications Required:
Personal qualities
• Languages: Bilingual Spanish/English desired
• Interest in and understanding of the challenges facing older adults, adults with disabilities, family caregivers, and individuals seeking to remain independent in their homes and communities
• Excellent organizational, interpersonal, and communication skills
• Self-motivated and self-directed, as most of the work will be done from his/her home office
• Ability to treat each client, volunteer, staff team member and members of the public with dignity and respect
• Experience working with Westchester County older adults and the resources available for them
Technology
• Excellent computer skills in MSWord, Excel, Outlook, e-mail and messaging
• Willingness/ability to master AssistedRides, the TRA volunteer ride management information system, and VisionLink, the resource information and referral system
Education & training
• Bachelor’s degree required in social work, public health, community organizing, or similar program
• Willingness to obtain Inform USA Aging & Disabilities Certification at TRA expense within 3 months of hire
Home office: Furnished home office with broadband internet connection
Salary and Benefits
· Minimum $40,000 to Maximum $60,000, depending on experience and credentials
