Job description

The Case Manager is responsible for overseeing resident care, coordinating services, supporting direct care staff, maintaining regulatory compliance, and ensuring the health, safety, and well-being of individuals receiving services in the group home. This position encompasses operational oversight, service coordination, and documentation responsibilities. The Case Manager serves as the primary point of contact for residents, guardians, interdisciplinary teams, and the Arizona Division of Developmental Disabilities (DDD).

ESSENTIAL DUTIES & RESPONSIBILITIES
  • Ensure residents receive personcentered care consistent with Individual Support Plans (ISPs) and behavioral support plans.
  • Monitor residents’ physical, emotional, behavioral, and medical needs.
  • Coordinate medical appointments, therapies, transportation, and community activities.
  • Communicate regularly with guardians, families, support coordinators, and clinical providers.
  • Assist with admissions, transitions, and discharges.
  • STAFF OVERSIGHT & COORDINATION
  • Supervise and support Direct Support Professionals (DSPs) during assigned shifts or ongoing operations.
  • Ensure staff are properly trained on residentspecific care plans, medication procedures, incident reporting, and safety protocols.
  • Monitor staff performance and provide coaching, guidance, and corrective action recommendations as needed.
  • Assist with staffing coordination and shift coverage when necessary.
  • COMPLIANCE & DOCUMENTATION
  • Maintain compliance with Arizona DDD regulations, licensing standards, and agency policies.
  • Review and maintain accurate resident records, progress notes, incident reports, medication logs, and documentation.
  • Ensure timely completion of required reports and documentation.
  • Participate in audits, inspections, and quality assurance activities.
  • PROGRAM COORDINATION
  • Coordinate implementation of habilitation goals and individualized programming.
  • Monitor resident progress and communicate updates to interdisciplinary teams.
  • Assist with development and implementation of behavior support and risk management strategies.
  • Promote resident independence, community integration, and quality of life.
  • HEALTH & SAFETY
  • Ensure the home environment remains clean, safe, and compliant with health and safety standards.
  • Respond appropriately to emergencies, incidents, and crisis situations.
  • Ensure proper medication administration procedures are followed.
  • Report incidents, injuries, allegations, or safety concerns according to policy and DDD requirements.
  • ADMINISTRATIVE RESPONSIBILITIES
  • Attend team meetings, trainings, and case conferences.
  • Maintain confidentiality in accordance with HIPAA and agency standards.
  • Assist leadership with operational tasks, scheduling coordination, and program improvement initiatives.
  • Perform other duties as assigned.
  • MINIMUM QUALIFICATIONS
  • High school diploma or GED required; Associate’s or Bachelor’s degree in social work, psychology, human services, or related field preferred.
  • Minimum of 2 years’ experience working with individuals with developmental disabilities.
  • Previous supervisory, lead staff, or coordination experience preferred.
  • Knowledge of Arizona DDD standards and regulations preferred.
  • Valid Arizona driver license with acceptable driving record.
  • Current CPR/First Aid certification (or ability to obtain upon hire).
  • Ability to pass background checks and meet DDD provider requirements.

  • Ability to lift, transfer, and assist residents as needed.
  • Ability to stand, walk, bend, and respond to emergencies.
  • Ability to work flexible hours, including evenings, weekends, holidays, or oncall rotation as needed.
