Requirements
- Active Registered Nurse (RN) License in State (or active NLC compact license)
- Valid driver's license, reliable transportation, and auto insurance
- Strong assessment and critical thinking skills
- Comfort with documentation and EMR systems (HCHB preferred)
Roles & Responsibilities
- Perform comprehensive patient assessments.
- Develop and update individualized plans of care.
- Provide skilled nursing care and appropriate interventions.
- Monitor patient progress and report changes to the care team.
- Educate patients and families on medications, treatments, and care plans.
- Coordinate care with other healthcare professionals.
