Job Details � Certified Nurse Practitioner
- Identify and manage at-risk patient populations, providing comprehensive care to optimize health and improve outcomes.
- Conduct thorough patient histories and physical exams (PMH and HPI) with precise documentation.
- Collaborate with multidisciplinary teams to develop personalized care plans.
- Utilize local medical and community resources to enhance the effectiveness of care interventions.
- Order, interpret, and act on laboratory results, EKGs, and various radiological studies.
- Design and deliver patient-specific educational materials tailored to individual needs.
- Stay informed of best practices in perioperative medicine and share updates with team members and other departments.
- Provide clinical leadership, guiding care teams to achieve optimal patient outcomes.
- Identify opportunities for process improvements and work with key stakeholders to implement changes.
- Actively participate in organizational committees to contribute to continuous care and process enhancement.
