Key Responsibilities
  • Conduct comprehensive assessments of patients with functional limitations, developing individualized treatment plans based on patient needs and clinical best practices.
  • Consult with healthcare providers, ensuring a collaborative approach to each patient's continuum of care.
  • Implement treatment plans, monitor patient progress, and adjust as necessary to maximize therapy effectiveness.
  • Document each patient's evaluations, treatment sessions, and outcomes accurately, adhering to all relevant regulations and maintaining compliance with industry standards.
  • Facilitate ongoing communication with patients, their families, and the interdisciplinary team, ensuring everyone is informed about progress and discharge plans.
  • Participate in staff meetings and continuous professional development opportunities, including training sessions focusing on advanced therapeutic techniques and patient care methodologies.
  • Supervise and mentor assigned support staff, aiding in their professional growth and the delivery of exceptional patient care.
Patient Population / Client Focus

You will primarily work with a diverse patient population, including individuals recovering from orthopedic surgeries, those managing chronic pain, and patients with neurological conditions. Your role will be integral in facilitating rehabilitation across various demographics, addressing the unique needs of patients from different cultural backgrounds, and maintaining a high standard of customer service.

Qualifications
  • Bachelor's Degree in Physical Therapy from an accredited program.
  • Current state license in Physical Therapy.
  • Demonstrated ability to communicate effectively both verbally and in writing.
  • Strong organizational skills and proficiency in word processing and electronic health record systems.
Preferred Experience
  • A minimum of 2 years of clinical experience in a physical therapy setting is preferred.
  • Experience with interdisciplinary care teams and familiarity with community outreach programs to enhance patient services.
Compensation and Benefits

Stone Crest Physical Therapy offers a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience and qualifications.
