Qualifications for Nurse Case Manager

• Current unencumbered State of Massachusetts RN license
• Knowledge of Medicare, Medicaid, PPS, PDPM, MDS and third-party payment
• Knowledge of regulatory agencies/licensure standards
• Must have minimum 5 years’ experience working in geriatrics with subacute experience
• Must have strong clinical background
• Case management experience strongly preferred

Responsibilities for Nurse Case Manager

• Monitoring level of HMO patients on admission and throughout stay
• Obtaining prior approval for all HMO patients throughout the facility for transportation, therapy, etc.
• Tracking HMO and Medicare days
• Obtain pre-authorization as needed for special services and equipment
• Ensuring sound clinical and financial outcomes
• Attending family meetings
