This role operates within the home care setting, enabling our therapists to provide services directly in patients’ homes. Key Responsibilities

As a Home Care Orthopedic Physical Therapist, your responsibilities will include:

• Evaluating patients recovering from injuries, surgeries, or chronic conditions within the home environment.
• Developing personalized treatment plans that incorporate a variety of therapeutic techniques including exercise routines, stretching maneuvers, and hands-on therapy to alleviate pain and improve mobility.
• Tracking and documenting patient treatments, responses, and progress using our electronic health record system, ensuring continuity of care.
• Regularly reassessing patient goals, adjusting treatment plans as necessary to reflect changing needs and progress.
• Fitting and recommending appropriate assistive devices to support self-care and ambulation.
• Educating patients, caregivers, and family members on therapy techniques, the proper use of assistive devices, and other relevant healthcare information.
• Maintaining current professional knowledge through continued education and participation in workshops or professional networks.

Patient Population / Client Focus

Your patients will include a diverse mix of individuals ranging in age from young children to older adults, each with unique orthopedic needs such as post-operative rehabilitation, sports injuries, or chronic musculoskeletal disorders. As a Physical Therapist, you’ll play a crucial role in transforming their recovery journey by instilling confidence and independence.

Qualifications

The ideal candidate will possess:

• A Doctorate in Physical Therapy from an accredited program.
• A valid state license to practice physical therapy.
• Excellent written and verbal communication skills.
• Proficiency in word processing and computer skills for documentation and communication.
• A flexible and adaptable approach to patient care, able to work independently and efficiently in home environments.

Preferred Experience

No prior experience is required, as new graduates are encouraged to apply. However, a strong understanding of orthopedic rehab principles will be beneficial. All candidates should demonstrate a genuine commitment to patient care and a desire to promote recovery and health.

Compensation and Benefits

We offer a competitive salary range of $100,000 to $120,000 annually, complemented by a comprehensive benefits package that includes:
