About the Role
Shodair Children’s Hospital is seeking a dedicated Care Management Licensed Practical Nurse (LPN) to coordinate patient admissions, discharges, and transfers across our Acute and Residential units. This role ensures seamless transitions of care, supports patient and family education, and upholds the highest standards of safety and compassion in every interaction.
Key Responsibilities
The ideal candidate is a compassionate, adaptable LPN with strong clinical and organizational skills. They communicate effectively across disciplines, stay calm under pressure, and use trauma-informed care to support patients through transitions. A collaborative team player, they balance independence with sound judgment and are passionate about helping children and adolescents heal and grow.
