Case Manager II
Pay: $24.24-$27.24/hour
Schedule: Monday through Friday 8am-4pm

Seven Hills Rhode Island is seeking a Case Manager II to oversee the day-to-day coordination of Shared Living/Adult Family Care (AFC) supports. This role ensures individuals receive high-quality, person-centered services that promote independence, choice, health, safety, and overall well-being while meeting regulatory and program requirements.

What You'll Do:
  • Coordinate, schedule, and monitor services for individuals and ensure their support needs are met.
  • Provide case management and coordinate medical, clinical, community, and other support services.
  • Develop and implement Person-Centered Plans, Individual Support Plans, and Individualized Support Agreements.
  • Advocate for individuals' rights, choices, independence, privacy, and personal goals.
  • Coordinate meetings with individuals, families, funding agencies, providers, and interdisciplinary team members.
  • Monitor service quality and ensure compliance with agency, state, federal, and funding-source requirements.
  • Maintain accurate, timely, and confidential case records and documentation.
  • Develop and monitor program goals, activities, staffing needs, and service schedules.
  • Provide guidance, education, and support to individuals, families, and staff as needed.
  • Interview, orient, train, supervise, and evaluate assigned staff.
  • Monitor budgets, staffing, training, and program resources to support effective service delivery.
  • Respond appropriately to health, safety, behavioral, and other urgent concerns.
  • Ensure individuals receive appropriate medical care, medication support, and other services as required.
  • Participate in quality improvement initiatives, team meetings, training, and professional development.
  • Perform other duties as assigned.
