Job description

Primary Purpose of Job

The Service Assessor is responsible for the completion of Medicaid Reassessments assigned to SeniorAge, primarily in Greene and the surrounding counties. This position works closely with others to further the mission, vision, and values of SeniorAge to ensure that participants have a positive experience. This position is a safety sensitive position. The is a remote position but will be making monthly home visits.

ESSENTIAL FUNCTIONS AND PERFORMANCE AREAS
  • Schedules in-home or phone visits with current HCBS recipients.
  • Completes InterRAI assessment tool, care plan, case notes and all required correspondence and documentation.
  • Completes reassessments by the last day of the month in which participant is due for reassessment.
  • Inform Service Quality Department with updates or issues regarding assessment participants.
  • Maintain excellent communication and professionalism when corresponding with participants and staff.
  • Protects participants rights by maintaining the confidentiality of information.
  • Provides excellent customer service to participants and staff.
  • Accurately and timely records information into the Web Tool system.
  • Other duties as assigned by the Service Assessor Lead.

    • Proficient in all Google applications, including Docs, Sheets, Drive and Gmail.
    • Skill in completing multiple tasks at once.
    • Ability to use good judgment and decision-making skills.
    • Ability to pay close attention to detail.
    • Possess excellent time management skills and strong communication skills (interpersonal, verbal and written).
    • Ability to work in a fast-paced, team environment with frequent interruptions.
EDUCATION, TRAINING AND EXPERIENCE REQUIREMENTS
  • All assessors must meet one of the following experience or education requirements:
  1. A bachelor's degree from an accredited college or university;
  2. A Registered Nurse (RN) who is licensed and in good standing in Missouri;
  3. A Licensed Practical Nurse (LPN) who is licensed and in good standing in Missouri with one or more years of experience working as an LPN;
  4. One or more years of experience as a Social Services Specialist;
  5. Four or more years of experience with the Division of Senior and Disability Services (DSDS) or an Area Agency on Aging; OR
  6. Multilingual and approved through DSDS.
  • Previous office experience required.
  • Must possess a valid driver's license and have a good driving record.
  • Strong understanding of the Older Americans Act.
