SUMMARY:

SafeinHome is seeking a compassionate, motivated, and detail-oriented Entry-Level Case Manager to join our team. In this role, you will work with a cross-functional internal team consisting of Remote Supports Consultants and Field Service Specialists and serve as the internal operations point-person for supporting those whom we serve. In addition, you will interact regularly with individuals, their families, and interdisciplinary support teams to develop and manage personalized support plans that reflect each individual's goals, preferences, and abilities.

As a Client Services Rep I, you will be in direct communication with Case Managers and Support Circle Teams to ensure the onboarding processes of individuals. You will also support Supervisors and Managers through a variety of tasks and be responsible for confidential and time sensitive material. Must learn, maintain familiarity with a variety of the field's concepts, practices, and procedures. You must have the ability to effectively communicate via phone and email and ensure that all duties are completed accurately and delivered with high quality and in a timely manner.

KEY RESPONSIBILITIES including, but not limited to:

• Collaborate with SafeinHome personnel, clients, families, and support teams to assess needs and develop individualized support plans
• Monitor, evaluate, and adjust support plans to ensure ongoing alignment with client goals and progress
• Advocate for clients' rights to autonomy, independence, and self-determination
• Maintain accurate and up-to-date case notes, documentation, and reports
• Coordinate with service providers, agencies, and other stakeholders to ensure continuity and quality of support
• Participating in team meetings, training, and professional development activities
• Uphold ethical standards and regulatory compliance at all times
• Independently manage their own onboarding pipeline
• Independently manage their own client base
• Read and understand the Person-Centric Individualized Service Plan (PCISP) and create Support Plan
• Communicate with Case Managers and Support Circle team members via phone and email
• Ensure that clients and their Support Circles are satisfied and identify the issues, documents, & reports when they are not satisfied
• Organize and schedule meetings and appointments on calendar
• Produce and distribute correspondence memos, letters, and forms
• Filter emails, make a task list, prioritizes the list, and complete the tasks in a timely manner
• Maintain computer and manual filing systems
• Confirm operational readiness of all equipment prior to initiating support activities
• Regularly join and contribute to Support Circle meetings
• Regularly perform support plan updates
• Ensure that Remote Support Specialists review the Support Plan and provide documented acknowledgment (sign-off)
• Calendar Management; add individuals’ schedules to the calendar, make updates as schedules change
• System Alignment Review
• Obtaining Authorizations and entering them into the CRM system
• Converting CRM Leads to Billable Accounts
• Use and Understanding of Procedure Codes and Modifiers across all States
• Billing Follow-Up - which can include research, correcting errors, requesting additional authorizations from external sources, etc.
• Schedule 3-Week Onboarding Review.
• Perform quality assurance tasks as needed
• Responds quickly and practically to emergencies, crises, or unexpected problems as they arise
• Handle sensitive information in a confidential manner
• Adheres to and accurately promotes all Company Policies and Procedures
• Does NOT share gossip, inappropriate personnel or personal details, or otherwise incite employee dissatisfaction
• Is viewed by other Client Services employees as valuable, knowledgeable, and as a high- functioning Company resource
• Keeps Supervisor, and/or management, apprised of issues impacting your/our ability to provide excellent service
• Ensure the confidentiality of our Clients’ Protected Health Information (PHI) and Personally Identifiable Information (PII).
