Job description

Overview

We are seeking an experienced and motivated Director of Nursing (DON) to lead and oversee all clinical operations within our Ohio home care agency. This leadership role is responsible for ensuring high-quality patient care, maintaining compliance with Ohio Department of Health (ODH), Ohio Medicaid, Medicare, and other regulatory requirements, and supporting the professional growth of clinical staff.

The ideal candidate will have a strong background in home health care nursing, clinical supervision, staff development, and regulatory compliance. The Director of Nursing will play a vital role in promoting compassionate, patient-centered care while ensuring operational efficiency and clinical excellence throughout the agency.

Responsibilities

  • Provide leadership and oversight for all clinical services within the home care agency.
  • Supervise and support Registered Nurses (RNs), Licensed Practical Nurses (LPNs), STNAs, home health aides, and other clinical staff.
  • Ensure compliance with Ohio Department of Health regulations, Medicaid waiver programs, Medicare guidelines, HIPAA requirements, and agency policies.
  • Oversee patient admissions, care coordination, reassessments, supervisory visits, and discharge planning.
  • Review and approve individualized care plans to ensure patient needs are met safely and effectively.
  • Conduct regular chart audits and documentation reviews to maintain compliance and accuracy within Electronic Medical Record (EMR/EHR) systems.
  • Monitor incident reports, patient complaints, missed visits, and corrective action plans to ensure quality assurance standards are maintained.
  • Coordinate staffing schedules and clinical coverage to ensure continuity of care for all clients.
  • Provide ongoing clinical training, mentorship, and competency evaluations for nursing staff and caregivers.
  • Collaborate with physicians, case managers, social workers, and interdisciplinary teams to improve patient outcomes and satisfaction.
  • Maintain readiness for state surveys, audits, and accreditation reviews.
  • Lead quality improvement initiatives focused on patient safety, compliance, and overall agency performance.
  • Foster a positive and professional work environment that promotes teamwork, accountability, and compassionate care.

Qualifications & Experience

  • Active Ohio Registered Nurse (RN) license in good standing.
  • Minimum of 2โ€“3 years of nursing leadership or supervisory experience in home health care, hospice, or related healthcare settings.
  • Strong knowledge of Ohio home care regulations, Medicaid waiver services, Medicare standards, and clinical compliance requirements.
  • Experience with EMR/EHR systems, clinical documentation review, and care coordination.
  • Excellent leadership, communication, organizational, and problem-solving skills.
  • Ability to manage multiple priorities while maintaining high standards of patient care and regulatory compliance.
  • Knowledge of infection control practices, patient safety standards, and quality assurance processes.
  • Experience preparing for and responding to state surveys and audits preferred.
  • CPR certification required.
