What You'll Do
In this role, you will direct the daily activities of adolescent males in programming utilizing acceptable behavioral management principles. Must have the ability to exercise independent judgment in crisis situations.
Your responsibilities may includes:
  • Clinical Care & Nursing Oversight
    • Provide and coordinate day-to-day healthcare services according to accepted nursing standards and individualized care plans.
    • Assist in assessing, planning, implementing, and evaluating healthcare services.
    • Conduct ongoing monitoring of individuals' health status and identify changes in condition.
    • Ensure service recipients receive appropriate medical care and follow-up services.
    • Work collaboratively with interdisciplinary team members to address healthcare needs.
  • Care Coordination
    • Communicate changes in health status and physician orders to families, guardians, and interdisciplinary team members.
    • Coordinate implementation of physician recommendations, treatment plans, and therapeutic interventions.
    • Collaborate with therapists and clinical professionals to ensure care plans are followed appropriately.
    • Maintain accurate and up-to-date transfer and discharge documentation.
  • Medication Management
    • Oversee medication administration processes.
    • Review medication orders, MARs, physician orders, and treatment documentation for accuracy.
    • Assist in maintaining medication inventories and pharmaceutical supplies.
    • Monitor medication storage and complete required monthly documentation.
  • Medical Records & Documentation
    • Maintain complete, accurate, and timely medical records.
    • Ensure health documentation complies with organizational, state, and federal requirements.
    • Organize and safeguard confidential health information.
    • Monitor documentation practices to ensure regulatory compliance.
  • Staffing & Scheduling
    • Coordinate and schedule LPN coverage across assigned homes and shifts.
    • Develop and maintain monthly nursing staffing schedules.
    • Submit nursing staffing plans for approval by the RN Area Manager and ICF Director.
    • Assist in addressing staffing needs and coverage concerns.
  • Training & Staff Support
    • Assist in developing and delivering health-related staff training.
    • Educate staff on medical diagnoses, medication side effects, activities of daily living support, and nursing protocols.
    • Support competency development and clinical best practices.
    • Serve as a clinical resource for direct support and nursing staff.

Qualifications
Education and Experience Required:
    • Current Licensed Practical Nurse (LPN) license in the State of Tennessee.
    • License must remain active and in good standing.
  • Experience
    • Minimum of two (2) years of clinical nursing experience required.
    • Three (3) to five (5) years of nursing experience in long-term care, residential services, home health, or related settings preferred.
    • Experience supporting individuals with intellectual and developmental disabilities preferred.
    • Knowledge of ICF/IDD standards and regulatory requirements strongly preferred.
  • Additional Requirements
    • Valid Tennessee, Arkansas, or Mississippi Driver's License with F Endorsement.
    • Acceptable driving record.
    • Ability to travel between service locations as neede
